UNIGROUP CERTIFICATE CHANGE – 30 DAY NOTICE OF CANCELLATION ENDORSEMENT
 
Beginning in January 2012, Unigroup will require a 30-day notice of cancellation endorsement to accompany certificates of insurance.  Our office has been in direct communication with Unigroup’s Risk and Insurance coordinator and received approval for the following endorsements.  Please use the approved wording below for Unigroup certificates based on the carrier providing insurance. Unigroup also confirmed that the 30-day notice of cancellation endorsement is not required until policy renewal. The 30-day notice of cancellation endorsement will be added in favor of Unigroup to accounts upon their renewal during 2012.
 
CHARTIS Certificate Language for Unigroup, Inc.

Unigroup, Inc. is added as additional insured with respect to general liability per CG2026 07 04, auto liability per CA2048 02 99, warehouse legal liability and cargo liability per the Moving and Storage Coverage Endorsement Form, for contract with insured; subject to all policy terms and provisions and legal liability established in the agent agreement.  30-day notice of cancellation provided per attached form 108538. (If WC coverage is written through our office with Zurich add: Coverage includes all states coverage and casual labor is included for all states except for the monopolistic states: OH, ND, WA and WY)
 
 
Attach the following forms when sending the certificate to Unigroup:

• Auto LiabilityDesignated Insured CA20 48 02 99
• Cargo/Warehouse Additional Insured Endorsement - Moving and Storage Coverage Form Additional Insured
• 30 Day Notice of Cancellation 108538 Limited Advice of Cancellation to Scheduled Entities

ZURICH Certificate Language for Unigroup, Inc.

Unigroup, Inc. is added as additional insured with respect to general liability per CG2026 07 04, auto liability per CA2048 02 99, warehouse legal liability and cargo liability per U-CIM-199-A-CW, for contract with insured; subject to all policy terms and provisions and legal liability established in the agent agreement.  30-day notice of cancellation provided per attached form U-GU-298A-CW (02/92). (If WC coverage is written through our office with Zurich add: Coverage includes all states coverage and casual labor is included for all states except for the monopolistic states: OH, ND, WA and WY)

NOTE:  It is your responsibility as the broker to provide a ‘CANCELLED’ certificate to Unigroup, Inc. upon notice of cancellation from the carrier.
